CONDITIONS FOR PARTICIPATION III.2.1) Personal situation of economic operators, including requirements relating to enrolment on professional or trade registers: Information and formalities necessary for evaluating if requirements are met: A1. A declaration that the applicant has not been the subject of a conviction by final judgment of: – participation in a criminal organisation, as defined in Article 2(1) of Council Joint Action 98/733/JHA(20); – corruption, as defined in Article 3 of the Council Act of 26 May 1997(21) and Article 3(1) of Council Joint Action 98/742/JHA(22) respectively; – fraud within the meaning of Article 1 of the Convention relating to the protection of the financial interests of the European Communities(23); – money laundering, as defined in Article 1 of Council Directive 91/308/EEC of 10.6.1991 on prevention of the use of the financial system for the purpose of money laundering(24). III.2.2) Economic and financial capacity: B1. Ownership and corporate form. If the applicant is a subsidiary, the name, address and registration number of the parent company and (if applicable) the ultimate parent company must be stated, as well as any information on the name and address of a Danish representative. B2. Name and address of the applicant’s bank. B3. Annual accounts for the previous three years and if 50 % (or more) of the company is owned by another company, the same information is desired concerning the parent company and (if applicable) the ultimate parent company. The accounting information shall include net turnover, result of primary operations, result after taxation, balance sheet total, equity ratio (equity capital as percentage of the total assets). B4. Documentation that the applicant can, in case of a contract, provide a bank guarantee by a bank acceptable to DONG Energy or guarantee commitment of 10 % of the contract value as security for meeting his obligation to deliver. III.2.3) Technical capacity: Information and formalities necessary for evaluating if requirements are met: C1. Statement of the applicant’s quality system according to ISO 9001 or similar. The same applies for major sub-suppliers. C2. Statement of the applicant’s environmental management organisation including principles for implementation of environmental management. C3. Statement of the applicant’s health and safety management organisation including principles for implementation of health and safety management. C4. Organisation chart of the applicant. C5. Statement of the applicant’s facilities for detailed engineering, planning and management of the works. C6. Statement of the applicant’s workshop and production facilities. C7. References of similar facilities, which the applicant has on order or has delivered within the last 5 years. The list of references shall include information for contact to owners of referred systems (name, address, telephone number etc.) and extent of work (disciplines involved, duration, place and value of contract).
