Travis Tappa, a 21-year-old worker for Voeller Mixer in Port Washington, Wisconsin, was working on one of the firm’s concrete mixers, with a crane working nearby. The crane made contact with a powerline, and Tappa was electrocuted. He was thrown around 20ft from where he was working, and suffered fatal injuries.
